Is Irby's injury Colt's fault?

I was at the game and I thought it then and after I saw the video I still think Colt made the wrong read and Irby's knee paid the price. If you look at the video you'll see that the left tight end was wide open a level behind Irby. McCoy hung him out there for the kill shot.
